Community Inclusion Service

At ConsultCare Solutions, we offer Community Inclusion Services to support individuals in engaging with their community through educational, enrichment, and recreational activities. These services are provided outside of a participant’s home in a small group setting of no more than six (6) individuals. Our goal is to promote social inclusion, skill-building, and personal growth through activities such as:

1. Team-building exercises to develop social and communication skills
2. Community outings including bowling, museum visits, and park trips

Volunteer opportunities to foster civic engagement and responsibility Each activity is designed to align with the participant’s Service Plan, ensuring meaningful experiences that enhance independence, social connections, and overall quality of life.
